Toolkit now available: A Guided Care Chronic Care Management Toolkit is now available that includes the Guided Care license, an implementation manual, 25 booklets for patients and families, one registration in the Guided Care Nursing online course, three registrations in the Practice Leaders in Medical Homes online course, and one hour of telephone consultation.

Free Training & Tools available to ACOs: Medical practices seeking to participate as accountable care organizations (ACOs) in the Medicare Shared Savings Program are eligible for free technical assistance on the Guided Care model. For details, read the press release or visit the Adoption Page.

Guided Care in PCMHs and ACOs: Medical practices and other organizations can adopt the Guided Care model for their chronically ill patients as they develop into patient-centered medical homes or accountable care organizations. Free technical assistance is available. Organizations could receive payment for providing Guided Care by participating in the Centers for Medicare and Medicaid Services Multi-payer Advanced Primary Care Initiative, the Federally Qualified Health Center Advanced Primary Care Practice Demonstration, the Comprehensive Primary Care Initiative, the Medicare Shared Savings Program as well as other initiatives in the private sector.

Utilization data published in Archives of Internal Medicine (Boult C et al.): After 20-months of a randomized controlled trial, Guided Care patients experienced, on average, 30% fewer home health care episodes, 21% fewer hospital readmissions, 16% fewer skilled nursing facility days, and 8% fewer skilled nursing facility admissions; only the reduction in home health care episodes was statistically significant. Guided Care produced even larger reductions in a subset of patients who received their primary care from one well-managed health system. Guided Care patients in Kaiser Permanente of the Mid Atlantic States experienced, on average, 52% fewer skilled nursing facility days, 47% fewer skilled nursing facility admissions, 49% fewer hospital readmissions, and 17% fewer emergency department visits; the differences for skilled nursing facility days and admissions were statistically significant. Caregiver data published in The Gerontologist (Wolff JL et al.): After 18-months of a randomized controlled trial, Guided Care caregivers judged the quality of chronic illness care received by their loved ones significantly higher than caregivers of patients of usual care. Guided Care caregivers who were employed (in addition to their caregiving role) reported increased work productivity, low absenteeism, and a decline in presenteeism (caregiving-related reductions in productivity at work).

Physician satisfaction data published in Annals of Family Medicine (Marsteller J et al.): After 1 year of a randomized controlled trial, primary care physicians who provided Guided Care reported higher levels of satisfaction with their patient/family communications and their knowledge of their patients' clinical characteristics than primary care physicians who provided usual care. Lipitz Center for Integrated Health Care in the Dept of Health Policy and Management at the Johns Hopkins Bloomberg School of Public Health has been selected as a finalist for the 2010 British Medical Journal Group's Getting Research into Practice Award for its work on Guided Care. Quality data published in the Journal of General Internal Medicine (Boyd CM et al.): After 18 months of a randomized controlled trial, Guided Care recipients rated their primary care significantly higher than usual care recipients with regard to coordination with specialists, support for self-management, and help received with setting goals, making decisions, and solving health-related problems. Guided Care patients were also more likely to rate as "excellent" or "good" the time they had to wait for an appointment when they were sick and their ability to get phone advice. Chad Boult has been awarded a Health and Aging Policy Fellowship funded by the Atlantic Philanthropies. Lipitz Center for Integrated Health Care at the Johns Hopkins Bloomberg School of Public Health received a $1.7 million grant from the John A. Hartford Foundation to help medical practices qualify for and participate in CMS's three-year Medicare Medical Home Demonstration project.
